Occasionally, a “patient” with hollowed-out eyes will start banging its head against the glass. Don’t approach and tell them to leave, because doing so can trigger a jump-scare and cost you 20 Sanity. A safer method is to give the NPC some coffee, which makes them leave without draining your Sanity.

If you want to avoid wasting the pot, use it efficiently: take two sips, then use the third for the head banger. After that, the head banger will eventually shatter the glass and exit on its own—provided you don’t go near it.

The breaking glass doesn’t change the overall gameplay, but it does stop the head banger from returning.

Twitchy Movements

Some patients will walk up to the counter and immediately begin twitching their arms or rolling their necks in an unsettling pattern. The moment you notice the behavior is off, hit the red shutter button to send the guest away.

Afterward, remember to open the shutters again so the next potential patient can enter.
