Roblox’s Animal Hospital turns every shift into a tense test of attention: anomalies are everywhere, and letting one slip through can spell disaster for you—and for your patients. How to spot anomalies in Animal Hospital

Noticing anomalies is easier to say than to do, but you have a few reliable detection methods. You can catch them through a patient’s appearance, through the patient’s photo, or by watching the CCTV feed. And while customers are the main concern, the hospital also contains multiple monster threats that you’ll need to recognize and respond to.

Because the game can punish you for missing something small, it’s important to check all the ways you can—especially before allowing a visitor or patient inside.

Appearance anomalies (no camera needed)

Some threats are visible right away and don’t require a camera or CCTV check. When you see the wrong signs, you can stop the interaction immediately by closing the shutters.

Look for the following appearance tells:

  • Twitching: Certain patients jerk their limbs erratically, especially around the arms and neck. They should be standing still, so watch carefully. Because the movement can be quick, observe the patient for at least four to five seconds to confirm they aren’t twitching.
  • Three eyes: Some patients have three glowing red eyes instead of the normal two, making this one of the most obvious appearance anomalies.
  • Hollow face: These patients have two empty eye holes and a wide, frowning mouth. Close the shutters and do not let them into the hospital.
  • Wide eyes and a creepy smile: Patients with large, wide eyes and a sinister grin can be identified immediately. Close the shutters right away—there’s no need to check CCTV.
  • Human teeth: Similar to the wide-eyes/sharp-teeth style threats, but with a mouth full of human chompers. Don’t process them—close the shutters immediately.
  • Mismatched eyes and sharp teeth: If a patient walks in with eyes positioned at different heights and sharp teeth visible in a menacing grin, send them away. Don’t risk your safety or your patients’ safety by letting them in.

Photo anomalies (the desk image tells the truth)

Other anomalies won’t show up just by staring at the patient. Instead, they appear in the photo. Some of these images can be cursed, and looking at them reduces your sanity by 10 points.

To avoid that penalty, take the patient’s photo and wait until it becomes clear on the desk. If it looks normal, you can pick it up.

Different eyes

If the photo shows different eyes than what you see on the actual patient model, it’s an anomaly. The discrepancy can be subtle—you might see that one eye area has eyelids or pupils while the other does not. The eyes don’t have to be completely different to count.

Different ears

Like the different-eyes issue, but instead the animal’s ears don’t match between the photo and the person in front of you. These differences can be more obvious than some eye mismatches, but still may require careful attention.

Incorrect photo

If the photo depicts a character model that doesn’t match the animal in front of you, treat it as an anomaly. That mismatch can involve different colors, different markings, or even a completely different animal.

Unnatural photo

Some photos show grotesque facial expressions that aren’t present on the patient standing before you. The guide specifically calls out possibilities like the human-tooth expression, uneven eye expression, a toothy grin, or a blurred picture. Close the shutters as soon as you spot this.

Static photo

Some photos contain a staticky film that obscures the image. That’s a sign of an anomaly, and you shouldn’t serve that animal—close the shutters to protect yourself and your patients.

Cursed photo

A photo is also an anomaly if it shows a character with bloodshot eyes, a grotesque smile, or blurred features. Looking at these pictures costs sanity, so avoid the debuff by observing photos on the desk before picking them up. There’s no reason to risk yourself by grabbing them immediately.

CCTV anomalies (only visible on the camera)

Some anomalies can’t be seen through the patient’s body or their photo; they only show up on the CCTV camera. That’s why you need to check every angle before letting anyone into the hospital. Some of these are also difficult to spot, so keep a close eye on the feed.

Even though cameras can break during a shift, the guidance is to fix the one you’re dealing with—so take the time to repair any camera issues. The waiting room camera gives you a view of the patient’s body, ears, tail, and face, and those are all the tells you need to spot an anomaly.

Censored eyes

Some figures have a large black rectangle covering their eyes, with another pair of eyes drawn on top. That censor box is an obvious anomaly signal and should be treated as such.

Unnatural body

If a patient or visitor has an elongated or otherwise unnatural body shape on CCTV, it indicates an anomaly. These figures can be extremely tall, very long, or wide, and their limbs may stick out at strange angles.

The hollow face expression that can appear in person or on photos may also show up on CCTV. It can be harder to detect, particularly if the anomaly isn’t looking toward the camera. Use the waiting room camera and closely check the patient’s eyes to confirm they aren’t hollow before letting them in.

Staring at the camera

Some anomalies stare directly at the camera when you view them on CCTV. A normal patient should be staring straight ahead at the office, not at the camera itself.

Void bodies

If a patient or visitor’s body is completely black on CCTV, it’s an anomaly. The lack of any discernible features makes this one relatively easy to spot.

Skinwalker

Skinwalkers show wide-open mouths with sharp teeth and long tongues hanging out. This may appear on CCTV, but it won’t show up in person or on photographs.

Mismatched ears or eyes

If the ears or eyes don’t match what the animal in front of you actually has, it’s an anomaly. This is described as the hardest type to spot because the camera is far away from visitors, and the only view of their eyes is in the waiting area.

The approach is:

  • First, look at the customer directly and note their ear and eye shape.
  • Then, check the CCTV feed to compare.

The difference can be tiny, such as an extra eyelid or a missing/extra pupil—so you need to examine the footage closely.

Twitching only on CCTV

Some patients or visitors may twitch on CCTV but not in person. That’s why you should watch the CCTV footage for at least four to five seconds. If you only watch for a second or so, it’s easy to miss the movement. This makes it one of the more difficult anomalies to catch.

Monsters in Animal Hospital

Customers aren’t the only threats. Animal Hospital also has multiple monsters roaming the premises. These enemies deal sanity damage when you get caught or make eye contact, which can decide whether you survive the run. Learning how to identify and deal with each monster is essential if you want to last as long as possible.

Bed Monster

The Bed Monster hides under the beds of patients and has large black hands pushing out beneath the mattress. There’s a large red box on the floor that you must avoid. If you touch that box, the monster will catch you, drag you under, and deal sanity damage.

Head Banger

The Head Banger is an unsettling anomaly with a hollow face. It enters the office, moves toward the window in the patient waiting area, and starts smashing its head into the glass. It will keep doing this for a couple of minutes, then crack the window and leave. It’s mostly inconvenient rather than deadly, because you can’t use the shutters until the monster is gone.

Ghost

The Ghost wanders through the hospital, phasing in and out, opening doors and scaring both players and patients. When you spot a ghost or notice ecotoplasma on the ground, grab a taser or gun. Sneak up behind it and use those items to get rid of it.

If you don’t know where the ghost is, get a teammate on a camera—because they can see its location in the hospital.

Skinwalker (as a monster threat)

Skinwalkers are monsters that emerge from anomalies after those threats have been allowed into the hospital. An anomaly can turn into a Skinwalker at any point after admittance. When it emerges, it lets out a roar, so keep sound enabled to get an early warning.

When you hear the roar, use a weapon such as a gun or taser and shoot the monster before it eats you or a patient. If needed, you can also fight it off with your bare hands.

Stalker

Stalkers are tall, black monsters that hide around corners and inflict sanity damage if you stare straight at them. If you spot one from the corner of your eye, avoid direct eye contact.

Tendril

Tendrils are large purple tentacles that emerge from patients in the surgery room. They’re extremely rare and only appear in Room 8. If you receive a warning that a monster has appeared in Room 8, go there and finish the patient’s surgery before the timer runs out—if you don’t make it in time, the patient will die.

Camera Anomaly

This anomaly shows up in broken CCTV cameras scattered around the hospital. When it spawns, the camera slowly zooms in on the monster. You must press the Exit button along the bottom of the screen in time to avoid sanity damage. Afterward, go fix the camera so you can look into it safely.

Don’t Look Up

The Don’t Look Up monster is described as a hideous, fleshy mass of skin and eyeballs that weeps on the ceiling. Avoid it simply by not looking up. If you do look up, it will deal steady sanity damage until you look away.

Hiders

Hiders are camouflaged monsters that conceal themselves against windows, curtains, and walls in the hospital. If you get too close, they break from their hiding spots and attack you.

They’re difficult to spot visually, but they breathe extremely loudly. Play with sound so you can hear their raspy breathing and scan the area where the noise comes from. You can still see their eyes and smile while they’re hidden.

If you spot one, either avoid it by not getting too close, or defeat it using your gun or taser.
